Faith Formation Registration for Grades 1-10 at Holy Trinity Parish can be completed using the form below. Please read all instructions and agreements prior to registering your child(ren). Elementary school classes are held at two locations (MBS and OLGC) subject to sufficient enrollment and teacher availability at each location. Insufficient enrollment or lack of teacher availability at a location may result in the merging of students for a grade with classes then held at a single location. If registering more than three children, submit an additional registration form.

CLASSES BEGIN SUNDAY, OCTOBER 2, 2022

FIRST COMMUNION AND CONFIRMATION REGISTRANTS

Preparation for First Holy Communion is a two-year program beginning in Grade 1. You may not register your child directly in Grade 2 for First Communion without completing Grade 1 or an approved make-up during the summer.

Preparation for Confirmation is a two-year program beginning in Grade 9. You may not register your child directly in Grade 10 for Confirmation without completing Grade 9 or an approved make-up during the summer.

IN ORDER TO BE ADDED TO THE CLASS ROSTER AND EMAIL DISTRIBUTION LIST.
REGISTRATION WITHOUT PAYMENT WILL AUTOMATICALLY BE PLACED ON HOLD.

- During the 2022-2023 school year, faith formation classes at Holy Trinity Parish will be held onsite subject to state, local, and archdiocesan pandemic guidelines and mandates. There is no virtual option for any grade.
  
  Grades 1 - 5 Elementary School
  The scheduling of classes is subject to enrollment and teacher availability. If there is insufficient enrollment at either location, students will be combined into one class and meet at a single location, Our Lady of Good Counsel or Most Blessed Sacrament. If classes meet at Our Lady of Good Counsel, the schedule is Sundays 9:15 - 10:15 AM two times a month according to the schedule published late September. If classes meet at Most Blessed Sacrament, the schedule is Sundays 9:30 - 10:30 AM two times a month according to the schedule published late September. No refunds are offered if classes are not held at the location you signed up for.
  
  Grades 6-8 Middle School
  Middle School students meet on Sundays 9:15 - 10:15 AM (two times a month) at Our Lady of Good Counsel Hall according to the schedule published late September.
  
  Grades 9-10 High School
  Confirmation classes meet on Sundays 11:30 AM - 12:30 PM (two times a month) at Our Lady of Good Counsel Hall according to the schedule published late September.
  
  Confirmation Attendance and Make-up Policy
  Students in both years of Confirmation (Grades 9 and 10) are required to attend ALL classes. Missing class (regardless of reason) is to be made up by completing a make-up assignment. Make-up assignments are intensive, time-consuming, do not necessarily reflect what was covered in class, and have a written component that must be returned by the due date. No more than 3 classes can be made up. Missing more than 3 classes will jeopardize the student's candidacy for Confirmation.

- Faith Formation Covenant
  I understand that the Faith Formation Program at Holy Trinity Parish is preparing my child(ren) for permanent and lasting participation in the Roman Catholic Church and commitment to the Gospel of Jesus Christ and the teachings of the Roman Catholic Church. I understand that as a parent/guardian I am committed to making sure that my child(ren) is (are) given both my personal example and my active support in learning their faith and making this commitment. This means that I recognize the importance of regular Mass attendance and the practice of my Catholic faith as an essential element to the success of this program in the life of my child(ren). I understand that if I have any obstacle to practicing my faith, either through personal concern or sacramental impediment that I am welcome to speak to the pastor at a reasonable time in order to work through any barrier to living the faith to which Christ has called me. I understand that a prerequisite for First Holy Communion and Confirmation is weekly attendance at Sunday Mass. There must be a commitment on my part to raise my children in the faith, to teach them to pray and to bring them to Mass as part of their sacramental preparation if they are to receive the sacraments of the Church.
  
  I understand that by participating in the faith formation program at Holy Trinity Parish, my child may be photographed during class activities, retreats, celebrations, or liturgical events, and these photographs may be posted on the official parish website.
  
  I understand that my registration is not complete until full payment is made and that an incomplete registration will be placed on hold (my child will not be added to the class roster and I will receive no program communication). By providing my email, I will be added to the Faith Formation email list for the communication of important program information and other events for the essential participation of my entire family in the sacramental and communal life of the parish.

- Classroom and Online Conduct
  The purpose of attending faith formation class is to come to know and share God’s love, one's faith and to strive to live the Gospel message in one's daily life. To build a closer relationship with Jesus and others is the goal of each class. Doing this, helps one become an active, caring member of the Holy Trinity Parish community. The following is the Code of Conduct contract that parents should familiarize themselves and their children with and agree to with this registration.
  
  1. I will bring my child to class on time. Attendance is recorded at each class. If I expect my child to be late to class, I will contact the Office of Faith Formation at htpreled@gmail.com.
  
  2. I will email or contact the Office of Faith Formation by phone in advance when my child will be absent from class stating the reason for the absence. If I attend Sunday class, I will contact my child's teacher. I will follow up with my child's teacher concerning missed work and have my child complete it within two weeks from the missed class.
  
  3. My child will come to class willing to participate and cooperate. S/he will treat her/his books with respect and complete any assigned work.
  
  4. S/he will behave appropriately by acting in a respectful manner to her/his teacher, staff, volunteers, as well as the other students. I understand that after three warnings by my child's teacher, I as parent/guardian will be contacted. I will then be required to sit in every class thereafter and observe my child for the remainder of the school year. If I cannot attend every class thereafter, or if my child's conduct continues to be disruptive or disrespectful, it will become necessary to remove my child from the program with the option to complete the remaining lessons at home.
  
  5. I will not permit my child to bring a cell phone or any other electronic device to class. If it is necessary to bring a device to class, the device will be handed over to the teacher for the duration of the class and returned to my child at the end.
  
  6. My child will respect all church property where classes take place. This includes the church, the parish hall, churchyard, parking lot, stairways, kitchens, bathrooms, bulletin boards, furniture, and wall decor.
  
  7. My child will not bring any food or drink into the premises where classes meet. On the occasions when my child's teacher permits food items to be brought to class for a celebration, I will not send in food that contains nuts of any kind.
  
  8. My child will request permission from her/his teacher before using the restroom facilities. Only one child may use the restroom facilities at a time.
  
  9. I agree to come into the halls where classes meet to drop off and pick up my child. My child will not leave the classroom prior to dismissal by her/his teacher. In the event s/he needs to leave prior to class dismissal, I will let the teacher know in advance and come to the class to pick up my child.
  
  I agree to abide by the above statements concerning my child's conduct in class, on church property, and at retreat locations. Violation of any of the above by my child will be reported to the Office of Faith Formation and to me.
